Please use the following form for making an appointment with Dr. Bushong. 

Here's an overview of the process:

• Authorize payment to Dr. Bushong.
  
  

• Choose how many sessions you want.
  
  

• Enter your phone number and suggest an appointment date.
  
  

• Complete the transaction.
  
  

• Dr. Bushong will contact you by email, usually within 12 to 24 hours, to confirm your appointment or offer you a choice of appointment times.
